Use this form if you are notifying a purchaser that the land contract with the purchaser will be forfeited for nonpayment toward the land contract unless the payments are made.
You must serve this notice on the purchaser. If you need a copy of the proof of service, click here.

If the purchaser does not make the payments as requested in the Forfeiture Notice, you can file a complaint (Form DC 103) with the district court requesting the court to enter an order forfeiting the land contract, awarding the seller possession, and evicting the purchaser.
