NOW COMES the above-named Debtor and states:

Pursuant to 11 U.S.C. § 109(h)(3), I am seeking a temporary waiver of complying with § 109(h)(1) of the Bankruptcy Code. I attest the following exigent circumstance(s) exists warranting the temporary waiver of my complying with § 109(h)(1):

Further, I requested credit counseling services from an approved nonprofit budget and credit counseling agency, but was unable to obtain the services referred to in § 109(h)(1) during the 5day period beginning on the date on which I made that request. Pursuant to 11 U.S.C. § 109(h)(4), I am seeking a permanent waiver of complying with § 109(h)(1) of the Bankruptcy Code. I attest this permanent waiver is based on my being:

incapacitated as that term is defined in § 109(h)(4) of the Bankruptcy Code; disabled as that term is defined in § 109(h)(4) of the Bankruptcy Code; or in active military duty in a military combat zone.
