HOW TO APPLY
1. Complete and sign application; print clearly. Give the complete description of the Collector vehicle in section A. Give the owner information in section B. Give the complete description of the vehicle, which is registered in Wisconsin and used for regular transportation in section C. If section C is not completed, the application is not acceptable. 2. If the vehicle that you wish to register with Collector plates is already titled in your name and: * Is currently registered, send a copy of your Certificate of Registration. * Is NOT currently registered, send the actual Certificate of Title - not a photocopy. 3. If the vehicle is NOT titled in your name, send a completed title application (MV1 or MV11) and fees, including Collector registration fees. 4. Using the fee schedule, determine Collector registration fees. NO credit is given from current registration. If you are transferring a Collector plate, you must pay the fee shown in the "Transfer" column. Make check or money order payable to: Registration Fee Trust. 5. GENERAL INFORMATION
Collector plates are nonexpiring for as long as you own the collector vehicle. Vehicles with Collector plates cannot be operated during the month of January unless a special temporary plate is purchased. Temporary plates are available for $5 for five consecutive days or $30 for the month of January, use form MV2750. To qualify for Collector plates, you must have at least one other motor vehicle, which is used for regular transportation, and is currently registered in Wisconsin, under the same name(s) as the Collector vehicle. Section C MUST be completed. If you already own a Collector plate, use "Transfer or Second and Subsequent Plate" fee schedule even when applying for a different vehicle type. For second or subsequent plates, allow approximately 4-6 weeks for the manufacturing process after the new Certificate of Registration has been issued. Truck-type vehicles with Collector plates cannot haul more than 500 pounds. To determine the approximate weight, use the weight of the vehicle with a full tank of gas and the equipment normally on the vehicle. Do not include the weight of a camper, lid or cap that is mounted on the truck or the weight of any persons or pets. By state law, Collector plates CANNOT be personalized.

RELEASE OF NONEXEMPT INFORMATION
The Wisconsin Department of Transportation uses the information on this form to issue Collector license plates. Under open records laws, the Department must make nonexempt information available upon request. Do you want your name and address withheld from mailing lists of 10 or more individuals? What qualifies?
All motor vehicles 20 years or older that have not been altered or modified from the manufacturer's specifications.

LEGAL OPERATION
To legally operate your vehicle, you must display license plates within two business days of purchase. If you do not have current plates, apply to a DMV Customer Service Center or an agent authorized by DMV to obtain a temporary plate while your Collector plate order is processed. An Agent may charge a $3 plate fee and/ or may also charge a $5 service fee.

NOTE: Street modified vehicles, replicas, homemade motorized vehicles or vehicles that have been altered or modified from the original manufacturer's specifications DO NOT qualify for Collector plates.
