DECLARATION RE: EX PARTE APPLICATION for TEMPORARY GUARDIAN This form may be neatly handprinted In support of the ex parte application the above named petitioner, or his/her attorney, declares as follows: 1. 2. (either) I am the attorney for petitioner. (or) I am the petitioner, (either) the parent(s), other party having legal custody of the child(ren), the party presently caring for the child(ren), or the attorney representing them, as noted in item number three (3) (indicate person by letter), below. I told them that I would be applying in writing to the San Joaquin Superior Court at the address noted above for an order appointing temporary guardian. (or) the household of the parent(s), other party having legal custody of the chid(ren), and the party presently caring for the child(ren) and the a person answering the phone at the attorney for such a party at the attorney's place of business. I was unable to get a phone number at which I could contact the person(s) listed in number three (3) (indicate persons by letter). I then left a message with the person answering the phone that I would be applying in writing to the San Joaquin Superior Court at the address noted above for an order appointing temporary guardian. I asked that the message be delivered as soon as possible. (and/or) I have not informed certain persons listed in item three (3) that I not be required to for the following reasons: (indicate person by letter) and request
