Instructions for Completing Renewal Application
If the mark has been changed or is no longer in use as registered or no longer used in the classification in which it was originally registered, the mark cannot be renewed. 1. Provide the name of the registrant/owner of the mark as it currently appears on the records of the Secretary of State's office. This information appears on the Renewal Notice as well as on the previously issued certificates; i.e., Certificate of Registration or any previous Certificate of Renewal for the specific mark. 2. Provide the business/mailing address where renewal correspondence should be sent. 3. Mark the appropriate box for the type of ownership of the mark as it currently appears on the records of the Secretary of State's office. 4. Complete item 4a or 4b only if the registrant is one of the business entities listed in item 3. 5. Provide the registration number assigned to this mark and the original date of registration with the Secretary of State's office. This is not the date of first use by the owner of the mark. 6. Provide the name of the mark as originally registered. 7. Provide the goods or services as named on the registration application. Signature and Contact Information The registrant or the attorney-in-fact for the registrant must sign the form. The person signing the form is declaring the mark to be in continous and current use in the normal course of commerce in the state of Illinois. Photocopied, stamped or faxed signatures are unacceptable. Specimens The renewal application must be accompanied by three examples of the mark showing actual use in commerce in the state of Illinois. For trademarks, examples may include the product label, business cards, letterhead or advertisements. For servicemarks, examples may include business cards, letterhead, brochures, flyers or advertisements. Name/tagline/logo printed, written or drawn on plain paper are unacceptable. If color is a feature of the mark, specimens reflecting the actual color(s) must be submitted. Duration and Renewal Registrations are effective for five years. If there is a change of address, it is the responsibility of the registrant to notify the Secretary of State's office. Renewals may be submitted no more than 60 days prior to the expiration date. The expiration date is the date of original registration.
