UNIFORM PROCEDURES FOR FORECLOSURE JUDGMENT PRESENTATION INSTRUCTIONS

Uniform Procedures for Foreclosure Judgment When submitting documents necessary to obtain a Judgment of Foreclosure to the Court always place them in the following order: 1. Completed Foreclosure Worksheet (JD-CV-77). 2. Signed copies of any open motion for default. 3. Affidavits of compliance with Emergency Mortgage Assistance Program (EMAP). 4. Military affidavits. 5. Original note, copy of the note for the court, mortgage deed, assignments, etc. 6. Updated affidavit of debt (note: Staple any updates to the top of the affidavit. Self-sticking adhesive notes with the updated debt are not accepted; any update must be permanently attached to the original affidavit of debt.) 7. Appraisal report (note: Staple the appraiser's affidavit to the top of the report. The affidavit should be executed by the person who conducted the appraisal and include the appraiser's fee.) 8. Itemized affidavit of attorney's fees.
