1. Your last three (3) years state and federal income tax returns. 2. The name, mailing address, and amount owed to any other secured or unsecured creditors. 3. Copies of all promissory notes and security agreements signed by you which are unpaid at this time. 4. Copies of all contracts for personal property and any UCC-1 filing statements securing the same. 5. Copies of all real estate deeds for property owned by you or any entity in which you have an interest. 6. Copies of all real estate mortgages owed by you at this time. 7. Copies of all car titles, truck titles, motorcycle titles, boat titles, etc. owned by you or any entity in which you have an interest. 8. An inventory of all business assets owned by you including the value of what you believe each item is worth (i.e. farm machinery and equipment). 9. The name and address of each financial institution in which you have a checking, savings or any other investment account. Identify the type of account and account number(s). 10. A detailed list of all personal property owned by you and the value of each item. 11. Copies of all financial statements prepared by you and filed with any of your creditors within the last five (5) years. 12. A list of any property which you claim is or should be exempt from the payment of your judgment indebtedness to the Plaintiff. 13. Copies of any other documents which reveal your financial situation (i.e. list of assets or debts).
