VERMONT ENVIRONMENTAL COURT 2418 Airport Road, Suite 1, Barre, Vermont 05641 (802) 828-1660 Docket No.
[Case title provided by the Court] [Docket Number provided by the Court]

Information if you are considering representing yourself in this case: Any person is entitled to represent himself or herself in a case before the Environmental Court, but not to represent anyone else. Representing yourself without a lawyer is sometimes called appearing before the Court "pro se ''. (The words ''pro se" mean "for self" in Latin.) If you want to represent yourself in this case, you should complete the written form at the bottom of this page and file this whole page with the Court. Be sure to include your complete mailing address and telephone number, so that the Court and the other parties can send you notices of court dates and copies of court papers. Each person must file a separate form, but you may mail them together in one envelope. You must send a copy of this appearance form to all the attorneys in the case and to all the other unrepresented (pro se) parties in the case. (You can call the court at 802828-1660 for these names and addresses.) You must also send a copy of anything else you file with the Court in the future to all the attorneys and to all the other unrepresented parties. You should also keep a copy for yourself. If your mailing address or phone number changes, please inform the Court and all the attorneys and other unrepresented parties. If you do not do so, you might not receive notice of a hearing or might not receive important papers from the other parties. You might lose the case if you do not come to a hearing or if you do not respond in writing to important papers. The staff of the Court will try to answer questions you may have about Environmental Court procedures, but they cannot give you legal advice. Everyone who comes before the Court must follow the same procedural rules, whether or not they are represented by an attorney. These rules are the Vermont Rules for Environmental Court Proceedings and the Vermont Rules for Civil Procedure; they are found in a volume of the Vermont Statues, which may be consulted in many libraries, or on-line.* Entry of Appearance for an Unrepresented ("pro se") Party I have read and understood the above information. Please enter my appearance on my own behalf ("pro se'') as an unrepresented party in this case. Dated at
