Rehabilitation plan service coding and categories Qualified rehabilitation consultants (QRCs) are required to use service codes and categories in the development of R-2 Rehabilitation Plan forms and R-3 Rehabilitation Plan Amendment forms. The service codes and categories must also be listed on vocational rehabilitation invoices for billing insurers, as prescribed by the commissioner. The service codes and service categories are listed below. After furnishing the service codes/categories, qualified rehabilitation consultants are to complete the rehabilitation plans by providing a description of the services. For example: For service code 03 ­ Coordination of RTW/same employer ­ the description might be, "Meetings or telephone conversations with employer to develop job opportunities." This is to be followed by the calendar start and end dates, and the projected cost for each listed service.
