INSTRUCTIONS: This part of the petition lists the categories being protested . Property Tax Assistance Division (PTAD) rules require that you also specify the value that the school district claims is correct . Attach all evidence regarding each change requested for the categories being protested to this sheet and include a hard copy of the (Protest and Recommendation) template and self report, if applicable . If you are not the district's designated agent, we must have written authorization from the agent permitting you to submit evidence (the agent may use the authorization at the bottom of this form to give this authorization) . You must submit to us the original and two copies of all evidence you want the division or hearing examiner to see . In the past, you used this sheet to give us the reason(s) that your requested changes made the study findings more accurate, which is required by PTAD rules . Beginning with protests of the 2008 Property Value Study, however, you will use the (Protest and Recommendation) (a separate document) to provide a reason that each requested change will make the findings more accurate .
