PUMP AND MOTOR EFFICIENCY TEST
(FOR ALL ELECTRIC PUMPS USE FORM DBB-019 (A))

COLORADO GROUND WATER COMMISSION Room 818 Centennial Building, 1313 Sherman Street Denver, CO 80203 NOTE: This form can only be used for wells located within a Designated Ground Water Basin.

This test is to determine the overall efficiency of the well (wire to water efficiency). All values, including pressure and input horsepower, must be measured on site. Note that well tests are performed several ways to obtain a variety of information. Providing a copy of this form to whoever does the test may help to insure that the correct procedure is followed. The test must be conducted by a certified well tester as determined by the State of Colorado. Provide a simple diagram of your irrigation system if you are now combining more than one type of irrigation (flood and sprinkler) or more than one sprinkler from the well. Locate existing meters, valves and pressure gauges. Include a brief explanation of your operating procedure. An example of a diagram is on the back of this form. Tests for the "expanded irrigated acres" application should be conducted late in the irrigation season, during August through October, when the ground water table is typically lower. Additional tests may be submitted at the applicant's option. The test shall determine a stabilized production rate and drawdown. This test must be conducted with the system under full operating conditions. If the well drives a sprinkler, the sprinkler must be connected and operating. A copy of the report of the person performing the test must be provided.
