DECLARATION AND REQUEST OF CONSERVATOR/GUARDIAN FOR ACCOUNTING WAIVER PURSUANT TO PROBATE CODE SECTION 2628(a)

I, _______________________________________, declare: 1. I was appointed Conservator/Guardian of the Estate on ____________________________________. 2. For the period beginning ______________________________ through __________________________, the Estate has met ALL of the requirements of Probate Code Section 2628(a) in that: a. the estate, at the beginning and end of the accounting period for which an accounting is otherwise required, consisted of property, exclusive of the residence of the conservatee/minor, of a total net value of less than $ 15,000.00; and b. the income of the estate for each month of the accounting period, exclusive of public benefit payments, was less than $2,000.00; and c. all of the income of the estate during the accounting period, if not retained, was spent for the benefit of the conservatee/minor. 3. Therefore, I request that the Court dispense with the need for an accounting for the period from __________________ through ____________________, and all future accountings provided the estate continues to meet the requirements of Probate Code section 2628(a) to waive an accounting. If my request is granted, I will biennially file a declaration that the estate continues to meet the requirements to waive an accounting, and will retain my records in the event the Court should, at some time in the future, require an accounting.
