To the Secretary of State of Idaho Pursuant to Section 30-1-1520, Idaho Code, the undersigned Corporation hereby applies for a certificate of withdrawal from the State of Idaho, and for that purpose submits the following statement: 1. It is incorporated under the laws of ___________________________________________________ . 3. It is not transacting business in the State of Idaho. 4. It hereby surrenders its authority to transact business in said state. 5. It revokes the authority of its registered agent in the State of Idaho to accept service of process and consents that service of process in any action, suit or proceeding based upon any cause of action arising in the State of Idaho during the time it was authorized to transact business therein may thereafter be made on it by registered or certified mail to the corporation at the address listed in item 6., below. 6. The post office address to which process against the corporation may be mailed is: ______________________________________________________________________________ . 7. It agrees to notify the Secretary of State of the State of Idaho of any change to the address in Item 6.

Line 1 - Enter the name of the corporation exactly as it reads in the records of the Idaho Secretary of State's Office. If the corporation was required to adopt a fictitious name to avoid a conflict with an existing business, or if the corporation had to designate a corporate ending to comply with section 30-1-401, Idaho Code, enter such name in the second portion of this section (the name which it used in Idaho). 2. Line 2 - Enter the domestic state of incorporation. 3. Line 3, 4 and 5 are statements which are required by Idaho Code. 4. Line 6 - Enter the address to which future service of process may be mailed against the corporation. 5. Line 7 is a statement which is required by Idaho Code. 6. The application must be signed by an officer of the corporation. Please specify the name of the signer by typing his/her name below the signature and indicate in what capacity he/she signs. (For example: President, Secretary, etc.) 7. Enclose the appropriate fee: a. b. c. d. If the form is typed, the filing fee is $20.00. If the form is not typed, the filing fee is $40.00.
