STATEMENT OF CHANGE OF DESIGNATED/PRINCIPAL/REGISTERED OFFICE AND/OR AGENT
Limited Partnerships must submit a Statement of Change form to change the Designated/Principal/Registered Office and/or Agent in Iowa. It is important to read these INSTRUCTIONS before you fill out the Change form. The numbers on these instructions correspond to the numbering on the form. PLEASE PRINT LEGIBLY.

Print the full name of the Limited Partnership on line 1.

Print the street and mailing address of the CURRENT designated/principal office.

Print the street and mailing adddress of the NEW designated/principal office.

Print the full name, street and mailing address of the CURRENT registered agent.

Print the street and mailing address of the NEW registered office, if applicable.

Form should be signed by a general partner. It must also state the date signed, the person's name, and the capacity in which the person signed.
