PLEASE TYPE OR PRINT LEGIBLY IN BLACK INK. 1. The company name must be listed as originally filed in the articles of organization or on the most recent amendment. 2. The company name must include the words or abbreviations Limited Liability Company, LLC, Professional Limited Liability Company or PLC, and may not include the words corporation or incorporated or any abbreviation of those words. The name will be recorded exactly as listed on this line. 3. The registered office address must be a Minnesota address and must be completed with a street address or rural route and rural route box number, city, state and zip code. A P.O. Box is not acceptable. 4. You are not required to have a registered agent. If you wish to have an agent now, you must list the full name of the agent who must be located at the registered office address. If you wish to remove a previously designated agent, write "none" in item 4. 5. If the duration is changing, list the desired duration of the LLC. This must be stated in years or can be listed as perpetual. 6. Write SAME if the business mailing address is the same as the registered office address listed in item 3. If the mailing address is not the same as the registered office address, state the mailing address. This address may be a P.O. Box. 7.
