COMES NOW PROBATION OFFICER Pascual Linarez presenting an official report on Maria
Guadalupe Gonzalez-ll/lacias who was committed to the Bureau of Prisons on March 7, 2005
(revocation), by the Honorable Robert C. Broomtield presiding in the District Court of Arizona. A 30-
month period of supervised release was re—imposed, and supervision commenced upon the offender's
discharge from imprisonment on I\/Iarch 7, 2005. In addition to the general terms and conditions adopted
by the court, the offender was ordered to comply with the following special conditions:
1. You shall participate as instructed by the probation ofEcer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. If deported, your shall not re-enter the United States without legal authorization and the express
written order/permission of this court.
3. You shall submit to search of person, property, vehicles, business, and residence to be conducted
in a reasonable manner and at a reasonable time by, or at the direction of, the probation officer.
Maria Guadalupe Gonzalez-Il/lacias was convicted of Count One, Importation of a Controlled Substance,
in violation of 21 U.S.C. § 952(a), a Class C felony.
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A. Violation of Special Condition Number 1: You shall participate as instructed by the
probation officer in a program ofsubstance abuse treatmentwhich may include testing for
substance abuse. You shall contribute to the cost of treatment in an amount to be
determined by the probation officer.
Gonzalez-l\/Iacias failed to participate in a program of substance abuse by failing to void urine
samples at the Foothills Walk-In Urgent Care in Yuma on October 6, 25, 31, November 17, 22,
and December 2, 2005; January 3, February 1, 9, 14, 24, I\/Iarch 2, 6, 17, 22, 27, and April 3, 14,
20 and 25, 2006. Grade C violation. § 7B1.1(a)(3).

B. Violation of Standard Condition Number 3: You shall report to the Probation Office as
directed by the Court or probation officer, and shall submit a truthful and complete written
report within the first tive days of each month.
1. Gonzalez-Nlacias failed to submit written monthly supervision reports to the U.S. Probation Office
for December 2005, and January, February and l\/Iarch 2006, which were due during the first five
days of each month. Grade C violation. § 7B1.1(a)(3).
2. Gonzalez-I\/lacias failed to report to the U.S. Probation Office on a monthly basis for supervision
as directed. She last reported for supervision on December 6, 2005. Grade C violation. §
7B1.1(a)(3).

AFFIDAVIT AND PETITION PRAYING THAT THE COURT WILL ORDER A WARRANT BE ISSUED
In conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, I am petitioning the Court to issue a warrant.
