INSTRUCTIONS FOR FILLING OUT PARENTING PLAN (Parent-Created) USE OF THIS PARENTING PLAN IS INTENDED ONLY FOR THOSE CASES IN WHICH ONE OF THE PARTIES PLANS TO SEEK FULL CUSTODY OF THE MINOR CHILDREN OF THE PARTIES. If you want things in your Parenting Plan besides those in this sample Parenting Plan, you can add them to your Parenting Plan. Likewise, if you do not want some things in your Parenting Plan that are in this sample Plan, you can take them out of your Parenting Plan. For example, you may want to add other holidays, take out particular holidays, change time periods, change the method of pick up and return times for visitation, change the length of time for summer visitation, etc. Also, if you observe holidays that are not included in this sample Plan, you can add language to your Plan about those holidays. Remember, this is your Parenting Plan and it should meet your individual needs.

BODY OF THE PARENTING PLAN First paragraph. This paragraph does not have a number. In the first blank, enter the plaintiff's full name. In the second blank, enter the defendant's full name. In the third blank, enter the case number assigned by the Clerk of the District Court when the Complaint was filed. In the fourth blank, enter the name of the county where the Complaint was filed.

The numbers or letters below give instructions for completing the paragraphs with the same numbers or letters in the Parenting Plan. Paragraph 7. Enter mother or father (custodial parent). Paragraph 8. Enter mother or father (custodial parent). Paragraph 9A. In the first blank, enter the time (including a.m. or p.m.) that weekend parenting time will begin. In the second blank, enter the time (including a.m. or p.m.) that weekend parenting time will end. In the third blank, enter the month, day and year when weekend parenting time will begin. Paragraph 9B. In the first blank, enter what day of the week (Monday through Friday) on which parenting time will take place. In the second blank, enter the beginning time (including a.m. or p.m.) that the parenting time will begin. In the third blank, enter the time (including a.m. or p.m.) that the parenting time will end. Paragraph 9C. (a) Easter. In the first blank, enter the time (including a.m. or p.m.) that Easter parenting time will begin. In the second blank, enter the time (including a.m. or p.m.) that Easter parenting time will end.

(b) Memorial Day Weekend. In the first blank, enter the time (including a.m. or p.m.) that Memorial Day weekend parenting time will begin. In the second blank, enter the time (including a.m. or p.m.) that Memorial Day weekend parenting time will end. (c) Fourth of July. In the first blank, enter the time (including a.m. or p.m.) that Fourth of July parenting time will begin. In the second blank, enter the time (including a.m. or p.m.) that Fourth of July parenting time will end. (d) Labor Day Weekend. In the first blank, enter the time (including a.m. or p.m.) that Labor Day weekend parenting time will begin. In the second blank, enter the time (including a.m. or p.m.) that Labor Day weekend parenting time will end. (e) Thanksgiving. In the first blank, enter the time (including a.m. or p.m.) that Thanksgiving Day parenting time will begin. In the second blank, enter the time (including a.m. or p.m.) that Thanksgiving Day parenting time will end. (f) Christmas. In the first blank, enter the time (including a.m. or p.m.) that Christmas parenting time will begin. In the second blank, enter the time (including a.m. or p.m.) that Christmas parenting time will end. (g) New Year's Day. In the first blank, enter the time (including a.m. or p.m.) that New Year's Day parenting time will begin. In the second blank, enter the time (including a.m. or p.m.) that New Year's Day parenting time will end. (h) Child(ren)'s Birthdays. In the first blank, enter the time (including a.m. or p.m.) that parenting time on the child(ren)'s birthday(s) will begin. In the second blank, enter the time (including a.m. or p.m.) that parenting time on the child(ren)'s birthday(s) will end. Paragraph 10. Father's/Mother's Day. In the first blank, enter the time (including a.m. or p.m.) that the father's and mother's parenting time with the child(ren) on Father's/ Mother's Day will begin. In the second blank, enter the time (including a.m. or p.m.) that the father's and mother's parenting time with the child(ren) on Father's/ Mother Day will end. Paragraph 11. Parents' birthdays. In the first blank, enter the time (including a.m. or p.m.) that the father's and mother's parenting time with the child(ren) on their birthdays will begin. In the second blank, enter the time (including a.m. or p.m.) that the father's and mother's parenting time with the child(ren) on their birthdays will end. Paragraph 12. Summer. In the first blank, enter the year that summer parenting time will begin. In the second blank, enter the length of time the summer parenting time will last, spelling out the number (i.e. one week, six weeks, ten weeks, etc.). Paragraph 13. Telephone. In the first blank, enter the day of the week (Monday through Friday) when telephone parenting time will occur. In the second blank, enter the beginning time (including a.m. or p.m.) of the telephone parenting time. In the third blank, enter the time (including a.m. or p.m.) that the telephone parenting time will end. In the fourth blank, enter the number of minutes each week the noncustodial parent will have telephone parenting time with the minor child(ren).

FINAL SIGNATURE If you are the mother, you will sign and date the document in the spaces provided on the left-hand side of the document. If you are the father, you will sign and date the document in the spaces provided on the right-hand side of the document. If your spouse is the mother, she should sign and date the document in the spaces provided on the left-hand side of the document. If your spouse is the father, he should sign and date the document in the spaces provided on the right-hand side of the document.



MAKE CERTAIN TO STAPLE THIS PARENTING PLAN TO THE DECREE OF DISSOLUTION THAT YOU TAKE TO YOUR FINAL HEARING.
