Affiant further states that __________________________ is the surviving joint tenant/ remainderman in the described property, and that the decedent is one and the same person as the joint tenant/ remainderman named in the deed recorded as described above. If this Affidavit is signed and acknowledged by the personal representative or duly appointed attorney in fact of the surviving joint tenant, certified copies of the letters of administration, letters testamentary, letters of guardianship or power of attorney for the Affiant must be attached.
