How To Apply
1. If you want personalized U.S. Veteran motorcycle (VMC) plates, choose 1-4 characters. If you want nonpersonalized VMC motorcycle plates, mark application and omit steps 2-4. If you choose 4 characters, no spaces are allowed. If you choose 3 characters or less, spaces are allowed. Indicate spaces with diagonal lines as shown here. 5. To order VMC plates, send: Completed application; Proof of eligibility; Issuance fee: nonpersonalized plates ($15.00) OR Personalized plate fee ($30.00 Even years or $15.00 Odd years) Registration fee if you do not have current plates. If the motorcycle is NOT titled in your name, also send: A completed title application (MV1 or MV11); and All fees including registration. Proof of Eligibility All documentation is subject to verification by the Department. Submit a photocopy of Department of Defense Form DD214, Certificate of Release or Discharge from Active Duty, or equivalent documentation indicating qualification as a veteran of the U.S. Armed Forces pursuant to Title 38, United States Code (Veteran's Benefits), Section 101.

RELEASE OF NONEXEMPT INFORMATION The Wisconsin Department of Transportation uses the information on this form to issue U.S. Veteran motorcycle license plates. Under open records laws, the Department must make nonexempt information available upon request. Do you want your name withheld from mailing lists of 10 or more individuals?

Nonpersonalized U.S. Veteran Motorcycle Plates Nonpersonalized U.S. Veteran plates, require: A $15.00 issuance fee; plus The biennial registration fee of $23.00 if the plates expire within the next 3 months.

U.S. Veteran Motorcycle License Plates Application
U.S. Veteran Motorcycle plates are issued to any Wisconsin resident who qualifies as a veteran of the U.S. Armed Forces pursuant to Title 38, United States Code, Section 101. The qualified individual must be an owner, lessee, or trust beneficiary.

Personalized U.S. Veteran Motorcycle Plates Personalized U.S. Veteran plates, require: Personalized plate fee $30.00 in even years OR $15.00 in odd years. If your personalized request is not available, check which option you would like us to follow. Biennial registration fee of $23.00 if you have no plates or if your current plates expire within 3 months. WIDOT may refuse to issue or recall after issuance, a request that is misleading or may be offensive to good taste or decency. No refund or adjustment will be made for a change of choice or spacing after the plate has been ordered, or if the application is unclear or incorrectly completed.

IMPORTANT NOTICE NO special parking privileges are provided with U.S. Veteran plates.
ADA - The Wisconsin Department of Transportation complies with the Americans with Disabilities Act.

Plates will be mailed 4-6 weeks after you receive a new Certificate of Registration.
