NO VIOLATIONS: At the time of inspection, no violations of 326 IAC 8-4-6 were observed. NO TRAINING PROOF: The observed violation of 326 IAC 8-4-6 is the failure to provide proof of annual Stage II training. In the future, please maintain Stage II training proof on site for at least one operator/employee regarding the proper operation of a vapor recovery system. FAILURE TO MAINTAIN A LOG(S): The observed violation(s) of 326 IAC 8-4-6 is the failure to maintain one or more of the following logs: daily inspection, maintenance, malfunction, and/or throughput. In the future, please maintain these logs and keep them on site. HOSE VIOLATION(S): The observed violation(s) of 326 IAC 8-4-6 is: the installation of an incorrect gasoline hose, the installation of a correct hose which is in poor operating condition, and/or the installation of a hose with an incorrect length. In the future, please install the correct hose in good operating condition and of the correct length for the Stage II system in use at the station. BELLOWS/FACE PLATE VIOLATION(S): The observed violation(s) of 326 IAC 8-4-6 is the failure to maintain the bellows and/or face plate in good working order. In the future, please maintain the bellows and/or face plate of all gasoline nozzles in good operating condition. NOZZLE ASSEMBLY VIOLATION(S): The observed violation(s) of 326 IAC 8-4-6 is: a loose clamp, In the future, please ensure that the Stage II nozzle and its components are in good operating condition. a defective swivel, or a leaking nozzle.

LOOSE RETRACTOR: The observed violation of 326 IAC 8-4-6 is a loose hose retractor. In the future, please ensure that all hose retractors are functional and keep the Stage II gasoline hose off the ground so as to avoid damage. FAILURE TO POST OPERATING INSTRUCTIONS: The observed violation of 326 IAC 8-4-6 is the failure to post Stage II operating instructions. In the future, please conspicuously post operating instructions for the Stage II system in the use at this gasoline dispensing facility. FAILURE TO SUBMIT REGISTRATION: The observed violation of 326 IAC 8-4-6 is the failure to submit a completed Stage II Registration form and accompanying test results. Please submit a completed Registration form and Stage II test results to the OAQ within thirty (30) days.
