PROGRAM HIGHLIGHTS
The Traffic Safety Program allows you to receive court supervision without a court appearance.

How does the program work?
When you participate in the Traffic Safety Program, you are granted court supervision without going to court. Court supervision is an unsupervised probation that lasts 180 days from the date of your violation, unless otherwise determined by a judge. Your responsibility is to remain without additional traffic violations during this time. What else do I have to do? Attend a ONE time, four hour class on defensive driving, developed by the National Safety Council and offered by Rock Valley College. You must complete this class within 160 days of the date of the violation. The Traffic Safety Program will notify the court when you have completed the class. It is your responsibility to maintain contact with the Traffic Safety Program and to successfully complete your class as part of your supervision. The Traffic Safety Program is NOT responsible for lost mail. How do I apply?

THIS TICKET WILL NOT APPEAR ON YOUR PUBLIC DRIVING RECORD IF YOU:
· Plead guilty but request court supervision and the Traffic Safety Program through the circuit clerk when you pay your ticket. · Pay your fine plus $30 for the program. · Complete an approved traffic class within 160 days. · Keep a clean driving record for 180 days.

When you receive a traffic ticket, you
have three choices:
1. PLEAD GUILTY and pay the fine, by mail or
in person, without going to court. A Conviction is reported to the Secretary of State.

2. PLEAD NOT GUILTY and request a trial. LEGAL ADVICE
Legal advice will not be given by the Office of the Circuit Clerk, the police department issuing the ticket or Rock Valley College.

THE TRAFFIC SAFETY PROGRAM

SUPERVISION. ATTEND THE TRAFFIC SAFETY PROGRAM. Pay the fine and school fee. Attend a one time class about defensive driving. Receive no additional tickets during your supervision period. Do not appear in court. NO CONVICTION WILL BE ENTERED ON YOUR PUBLIC DRIVING RECORD. 1 q

Check the eligibility guidelines and the registration information carefully before you complete this application. You must apply for the program through the circuit clerk's office when you pay your ticket. What happens after I apply? Your school registration will be sent to Rock Valley College by the office of the Circuit Clerk. The college will notify you by mail of the time, date and location of your class. The Traffic Safety Program is not responsible for lost or undelivered mail. If you are unable to attend the class to which you are assigned, contact the Traffic Safety Program IMMEDIATELY at (815) 921-3940 to reschedule. If you do not receive a letter of confirmation from the Traffic Safety Program, or a class assignment letter within 90 days from the date your ticket was issued, PLEASE CALL OUR OFFICE.

ELIGIBILITY GUIDELINES
Read these Eligibility Guidelines carefully to see if you qualify. If you apply for the program and are found to be ineligible, a conviction will be reported to the Secretary of State. Both your fine and school fee are non-refundable. 1. You may participate in the Traffic Safety Program to receive mail-in court supervision for ONE ticket a year. (The year is figured from ticket date to ticket date.) 2. Anyone who has already received court supervision (either by order of a judge, or by attending a Traffic School Program) for another traffic ticket that was issued within 12 months of this ticket, is not eligible. 3. If your ticket is marked "MUST APPEAR IN COURT," you are required to appear in court, in person, on the date shown. 4. If you have a CDL (Commercial Driver License) call our office before making this application To participate in the program, you must be willing to: 1. Plead guilty to the violation by signing your ticket and the enclosed "PLEA OF GUILTY" AND APPLICATION FOR COURT SUPERVISION FORM. 2. Pay the fine in full and the Traffic Safety Program fee. 3. Complete the traffic class within 160 days of the violation. 4. Keep a clean driving record during your 180 days of supervision.

REGISTRATION INFORMATION
1. Sign the copy of your ticket marked "court communication's copy" in the "Appearance Plea of Guilty and Waiver" section on the back, at the bottom. (Defendant's signature) 2. Complete and sign the "PLEA OF GUILTY" AND APPLICATION FOR COURT SUPERVISION FORM in this packet. (page ) 3. Complete the ROCK VALLEY COLLEGE TRAFFIC SAFETY PROGRAM REGISTRATION FORM in this packet. (page ) 4. Return all of the above to the Circuit Clerk's office, in the envelope provided, no later than 10 days before the date written on the front of the ticket, in the appearance section. 5. Include a bank order, certified check, or a personal check (for Boone, Lee & Winnebago County only) in the amount of your fine plus a $30 school fee, made payable to the Circuit Clerk. (See the chart below.) 6. If you have paid cash to the officer as your bond, pay only the school fee. If you have submitted a bond card, pay both the fine and the school fee. (Your bond card will be returned to you.) 7. Enclose a stamped, self-addressed envelope for return of your driver's license or bond card. Payment may be made in person or by mail. PERSONAL CHECKS WILL BE ACCEPTED by Boone, Lee, and Winnebago Counties ONLY. In-person payments may be made in cash. DO NOT MAIL CASH. If you have a CDL (Commercial Driver License) call our office before submitting this application.

OUT OF TOWN MOTORISTS
Rock Valley College administers a Traffic Safety Program for tickets issued in the following Illinois counties: Boone, Carroll, JoDaviess, Lee, Ogle, Stephenson and Winnebago. Classes are held in: (Rockford, IL, Belvidere, IL, Freeport, IL, Dixon, IL, Elizabeth, IL). Motorists who want to attend class at a non-Rock Valley location must: 1. Complete and submit this application as directed. You must include the $30 fee for the school supervision program. (Even if you elect to attend classes somewhere else.) 2. Call the Rock Valley College Traffic Safety Program to be referred to an approved traffic school. (You are responsible for your own registration and additional fees to this school.) 3.
