Gentlemen: I am hereby petitioning this Court for a hearing to Retain my Driving Privileges in accordance with Ohio Revised Code 4507.02.01. I reside within the jurisdiction of the Bedford Municipal Court, agree to pay all costs incurred, and have sufficient evidence to warrant the requested hearing. I am notifying the Bureau of Motor Vehicles by certified mail of this petition by sending them a copy of this letter and requesting a record of convictions. **Instructions: 1. 2. Complete an original and make three copies. File the original and one copy with the Court along with the filing fee. You will get a third copy time-stamped by the Court so you can send this copy to the BMV and keep a copy for yourself. Bring a copy of your notice of suspension to Court for filing. MVODIP P.O.Box 16520 Columbus, OH 43216 The Court will send you notification of your hearing date by mail, so make sure your address is correct and legible on your petition. YOU MUST HAVE INSURANCE INFORMATION FORM FILLED OUT BY YOUR INSURANCE AGENT AND BRING THIS WITH YOU TO YOUR COURT DATE. A COPY OF YOUR NOTICE OF SUSPENSION MUST BE FILED WITH THE ORIGINAL PETITION. You should be aware that if driving privileges are granted, they will only be considered valid upon completion of a remedial driving course and the filing of your SR-22 insurance bond with the BMV. Failure to complete these BMV requirements will invalidate any privileges granted by the court, so it may be a good idea to take your remedial driving course and file the SR-22 bond BEFORE appearing at Court.
