Parent respectfully represents: 1. The value of the assets of the conservatorship is: $_________________, which is less

than $10,000. A copy of the most recent account statement is attached. 2. Parent wishes to transfer the entire assets of Account #_______________________

Parent agrees not to request court approval to withdraw funds from the account, except

UTMA account. 4. Ordering Parent to provide the Court with an Affidavit and Acknowledgment of

Restriction from the financial institution holding the UTMA account, within 30 days of the state of the order approving this motion. 5. Authorizing the Conservator to be discharged and the file closed, upon filing of a

receipt of the conservatorship assets from the financial institution holding the UTMA account.
