Line 1 - Enter the name of the limited liability company exactly as it is filed with the office of the Secretary of State. 2. Line 2 - Enter the date the articles of organization were filed with this office. 3. Line 3 - List the reason for the dissolution. 4. Line 4 - If you have other comments to make regarding the dissolution of the limited liability company you may do so in this area. 5. The articles of dissolution must be signed by a manager of the LLC, if its articles of organization vested the management in managers, or by a member, if the management is vested in members.
