employee in the possession oi your company;
5, All work evaluations, performance reviews, disciplinary actions, complaints,
incident reports, accident reports or other data involving the employee;

7. A copy of any group insurance policy which provided medical, disability, and/or
reimbursement of loss of wages benefits tothe above»named employee;
8. All receipts, correspondence, and/or other documents indicating payments made
to the above—named employee or on his behalf for medical bills, and/or loss ol wages under any
group insurance policy provided through your company;
9, Any and ail information regarding workers’ compensation claims flied at any time

10. Any and all complaints, claims, charges or threatened complaints, claims,
charges made by or on behalf of the abov~named employee and any documents related
