Michael Meelhuysen, first duly sworn upon his oath states the
following:
1. I am now, and have been an officer with City of Phoenix Police
Department since July 3, 1999.
2. While on duty as an officer with the Phoenix Police Department, I
participated in the arrest of Robert Krumlauf on October 24, 2003
in a residential backyard located at 3151 West Ruth Avenue in
Phoenix, Arizona. This arrest is the subject of Department
Report No. 2003—32009463.
3. I am aware of the subject of the lawsuit entitled Krumlauf v.
Phoenix Police Oflicer Michael Meelhuysen, et al., United States
District Court No. CV 04-0720-PHX—ROS (LOA).
4. On October 24, 2003, at approximately 8:45 p.m., I noticed
Robert Krumlauf to be the front passenger in a vehicle that was
driving recklessly.
5. Following a short pursuit by myself and Officer Jeffrey Jakemer,
Robert Krumlauf bailed out of the vehicle at 3151 West Ruth
Avenue and fled on foot.
5. David Welland, the homeowner at 3151 West Ruth Avenue,
approached me and advised that there was a man lying in his
backyard and that he wanted police to contact the man.
7. I then entered the backyard and located Robert Krumlauf lying
beneath a bush.

8. I ordered Robert Krumlauf out from underneath the bush and he
complied by crawling out from underneath the bush.
9. At approximately 9:00 p.m., I placed Robert Krumlauf in
handcuffs.
10. Robert Krumlauf stated he had an injured ankle and could not
stand up or move. I-ie advised that he had injured his ankle when
he bailed out of the vehicle and then ran and jumped over the
fence into the backyard.
11. Robert Krumlauf identified himself as Robert Calouff and gave a
date of birth of March 3, 1967.
12. I noticed that the fence in question had been damaged. David
Welland advised that the fence had been damaged when Robert
Krumlauf jumped over it. I observed several wooden slats that
composed the fence had been broken. lVIr. Welland advised that
he was willing to prosecute.
13. I ran a records check with the information Robert Krumlauf
provided and found no record. I asked Robert Krumlauf again for
his identifying information and he gave me the same name but a
date of birth of June 6, 1967. I conducted another records check
and again found no record.
14. Robert Krumlauf was examined by Phoenix Fire Department
Paramedics from Engine 30. The paramedics advised me that he
had no serious injuries.
15. Officer Scelzo then escorted Robert Krumlauf to the backseat of
my patrol vehicle.
16. I transported Robert Krumlauf to the Phoenix Police
Headquarters for hngerprinting.
17. I provided Robert Krumlauf with a standard warning regarding
arrest for forgery for falsely signing a name to an official
document. Robert Krumlauf read the warning and I read it aloud
to him. Robert Krumlauf indicated he understood the warning.
18. Robert Krumlauf repeated that his name was Robert Clauoff and
signed that name to the fingerprint card.

19. I submitted the Hngerprint card to a Phoenix Police Department
Records and Identihcation Technician who advised me that
Robert Krumlaufs real name was Robert Krumlauf with a date of
birth of March 3, 1968.
20. I then advised Robert Krumlauf that I had discovered his real
name and he responded, “Well what the hell do you expect me to
do, once you’ve done time in DOC, you don't want to, uh,
nevermind."
21. I then transported Robert Krumlauf to the Madison Street Jail and
completed booking paperwork for forgery and criminal damage.
