Petition to Revoke Supervised Release/I:
COMES NOW PROBATION OFFICER Branden Austill presenting an official report on Douglas
Lyle Scott who was committed to the Bureau of Prisons on June 13, 2005 by the Honorable Susan R.
Bolton presiding inthe District Court of Arizona. Athree—year period of supervised release was imposed,
and supervision commenced upon the 0ffender's discharge from imprisonment on November 21, 2005.
In addition to the general terms and conditions adopted by the court. the offender was ordered to comply
with the following special conditions: .
1. You shall participate as instructed by the probation officer in a program of substance abuse
treatment which may include testing for substance abuse. You shall contribute to the cost of
treatment in an amount to be determined by the probation officer.
2. You shall submit your person, property (including but not limited to computer, electronic devices,
and storage media), residence, office, or vehicle to a search conducted by a probation officer, at
a reasonable time and in a reasonable manner.
3. You shall provide the probation officer access to any requested financial information.
4. You are prohibited from making major purchases, incurring new financial obligations, or entering
into any financial contracts without the prior approval of the probation officer.
5. You shall participate in a mental health program as directed by the probation officer which may
include taking prescribed medication. You shall contribute to the cost of treatment in an amount
to be determined by the probation ofhcer.
6. You shall not contact the victim, and the probation officer will verify compliance.
7. You are prohibited from owning, maintaining or using a firearm.
8. You shall not be involved with gang activity, possess any gang paraphernalia or associate with
any person afhliated with a gang.
9. You shall abstain from all use of alcohol or alcoholic beverages.
10. You shall cooperate in the collection of DNA as directed by the probation ofHcer.
Douglas Lyle Scott was convicted of violating Title 18, USC §1153 and 113(a)(7), CIR Assault Resulting
in Substantial Bodily Injury, a Class D Felony Offense.

RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A. Violation of Standard Condition #3: You shall report to the Probation Office as directed
by the Court or probation officer, and shall submit a truthful and complete written report
within the first five days of each month.
Scott did not submit monthly supervision reports for the following months: February, March, April,
and May 2006, which were due within the first tive days of each month. Grade C violation
§7B1.1(a)(3)
B. Violation of Standard Condition #7: You shall notify the probation officer at least ten days
prior to any change of residence or employment.
Scott did not report an updated change of residence to the supervising probation officer. On May
21, 2006, during a home contact this ofHcer was informed by the offenders girIfriend's friend,
Mickey, Scott had not resided at the home for nearly two weeks. She added, Scott returned to
his aunt's home. Grade C violation §7B1 .1(a)(3)
C. Violation of Standard Condition #19: You shall pay any monetary penalties as ordered by
the Court. You will notify the probation officer of any material change in your economic
circumstances that might affect your ability to pay restitution, fines, or special
assessments.
Scott did not pay the special assessment fee of $100 and has not made any payments towards
the assessment. Grade C violation §7B1.1(a)(3)
D. Violation of Special Condition #1: You shall participate as instructed by the probation
officer in a prog ram of substance abuse treatment which may include testing for substance
abuse. You shall contribute to the cost of treatment in an amount to be determined by the
probation officer.
1. Scott did not set an appointment for a substance abuse intake assessment at the CRIT
Adult Substance Alcohol Program as previously instructed by the supervising probation
officer on the following dates: November 29, 2005, January 10, 2006, February 7, 2006,
March 15, 2006, and May 26, 2006. Grade C violation §7B1.1(a)(3)
2. Scott did not set an appointment for placement in the random drug testing program with
the TASC Office as previously instructed by the supervising probation officer on the
following dates January18, 2006, March 15, 2006, and May 26, 2006. Grade C violation
§7B1.1(a)(3)

E. Violation of Special Condition #5: You shall participate in a mental health program as
directed by the probation officer which may include taking prescribed medication. You
shall contribute to the cost of treatment in an amount to be determined by the probation
officer.
Scott did not set an appointment for a mental health intake assessment at the CRIT Behavioral
Health Services office as previously instructed by the supervising probation officer on the
following dates: November 29, 2005, January 10, 2006, February 7, 2006, March 15, 2006, and
May 26, 2006. Grade C violation §7B1.1(a)(3)
AFFIDAVIT AND PETITION PRAYING THAT THE COURT WILL ORDER A WARRANT BE ISSUED
ln conformance with the provision of 28 U.S.C. § 1746, I declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, I am petitioning the Court to issue a warrant.
