16 1. The affiant is the Judgment Creditor, his personal
representative, or assignee, or a duly authorized agent of either, and is authorized to
17 execute this Judgment Renewal Affidavit on his behalf. I-
18 2. On June 11, 2002, Judgment in this action was granted against 1
19 RICHARD M. KUBANY.
3. The total amounts owed by Defendant on the date Judgment
26 was granted was $3,500,000.00 plus interest of ten percent (10%) per annum from
21 the date of entry until paid in full.
4. Judgment was entered by the Clerk of the Superior Court of
22 Maricopa at Letters "K" of Judgment Docket microfilm.
23 5. A Transcript or Certified Abstract of said Judgment and any
subsequent renewals were recorded in the offices of the County Recorders of the
24 below listed counties in the docket and at the page set forth below:

9 amount of $5,253,835.62 is due and owing on this Judgment.
10 11. This Affidavit is made for the purpose of renewing this Judgment
and renewing all Judgment Liens thereon in accordance with A.R.S. § 12-1612 and §
11 12-1613.
12 WITNESSETH that l have read the foregoing Affidavit and that l verify
the same of my own knowledge and positively, and not upon information and belief,
