Petition to Revoke Supervised Release
COMES NOW PROBATION OFFICER Michael A. Watts presenting an official report on Javier
SANDEZ who was committed to the Bureau of Prisons on January 10, 2005, by the Honorable Roger
G. Strand presiding in the District Court of Arizona. A 36 month period of supervised release was
imposed, and supervision commenced upon the offenders discharge from imprisonment on October21,
2005. In addition to the general terms and conditions adopted by the court in General Order 04-11, the
offender was ordered to comply with the following special conditions:
1. The defendant shall maintain full-time employment and/or schooling as directed bythe probation
officer.
2. You are prohibited from owning, maintaining or using a firearm. *
3. You shall cooperate in the collection of DNA as directed by the probation officer.
Javier SANDEZ was convicted of transportation of illegal aliens.
RESPECTFULLY PRESENTING PETITION FOR ACTION OF COURT FOR CAUSE
AS FOLLOWS:
A. Violation of Standard Condition 18: lf supervision follows a term of imprisonment, you shall report
in person to the Probation Office in the district to which you are released within seventy-two (72) hours
of release.
The offender was released from the Bureau of Prisons in Victorville, California, October 21, 2005, and
never reported to the Probation Department. His current whereabouts are unknown. Prior to release,
the offender signed a notice of release and arrival form which instructed him to report with 72 hours of
release.
This constitutes a Grade C violation. §7B‘I .1(a)(3)(B)

AFFIDAVIT AND PETITION PRAYING THAT THE COURT WILL ORDER A WARRANT BE ISSUED
in conformance with the provision of 28 U.S.C. § 1746, l declare, under penalty of perjury, that the
foregoing is true and correct to the best of my knowledge. Based on the information presented that the
offender has violated conditions of supervision, I am petitioning the Court to issue a warrant.
