The Court issued an order of protection against the respondent after finding that the respondent received reasonable notice and opportunity to be heard. This order was issued on ____________ and will expire on _________. Pursuant Tenn. Code Ann. § 36-3-625, the Court ordered the Respondent to surrender all firearms. Respondent understands that he/she is prohibited from possessing a firearm for so long as the order of protection or any successive order of protection is in effect AND that this form must be completed and filed with the Clerk of Court/Court within one business day from date of dispossession of the firearms or upon specific order of the Court as provided in the Court's order.

The Respondent: The respondent surrendered the firearms to a third party who is not prohibited from possessing firearms. The firearms were not registered under the National Firearms Act.



Possesses a weapon under the National Firearms Act that requires approval of any state or federal agency prior to the transfer of such firearm. The weapons are registered under the National Firearms Act and the respondent received state or federal agency approval to transfer the firearms to a third party who is not prohibited from possessing firearms or the firearms were placed into a safe or similar container which is securely locked and to which the respondent does not have the combination, keys or other means of normal access. Possesses firearms as a federal firearms dealer or a responsible party under a federal firearms license that constitute the business inventory under such federal license as determined based upon the applicable federal statutes or the rules, regulations and official letters, rulings and publications of the bureau of alcohol, tobacco, firearms and explosives. The respondent surrendered the inventory to a third party who is not prohibited from possessing firearms and a federally licensed firearms dealer or there was no surrender because there is another individual who is a responsible party under the federal license who is not the respondent subject to the order of protection.
