Affidavit for Transfer of Title to Motor Vehicles in a Small Estate Proceeding (1) (2) Being first duly sworn, I state that: This affidavit is made for the purpose of transferring title to motor vehicles owned by ____________________________________________________, who died on ___________________, as authorized by §75-3-1201; (a) I am the decedent's successor and entitled to payment or delivery of the property; (b) I am the duly authorized agent of __________________________, who is the decedent's successor; (4) The value of the entire estate subject to administration, wherever located, other than those motor vehicles, trailers, or semi-trailers described below, less liens and encumbrances, does not exceed $100,000; 30 days have elapsed since the death of the decedent; No application or petition for the appointment of a personal representative is pending or has been granted in any jurisdiction; and The decedent's successor is entitled to delivery of the motor vehicles, trailers, or semi-trailers described below. The Motor Vehicle Division of the Utah State Tax Commission shall transfer title of not more than four boats, motor vehicles, trailers, or semi-trailers, from the decedent to the successor or successors upon presentation of this affidavit and upon payment of the necessary fees.
