5. An action within the jurisdiction of the family division of circuit court involving the family or family members of the minor has been previously filed in assigned to Judge 6. Court, Case Number , and remains is no longer , was pending.

a. I am the prosecuting attorney for this county. I have been notified that an adoption petition has not been filed and the child has not been returned to the parent or person having the right to legal custody. b. I am the prospective adoptive parent. I am unwilling or unable to proceed with the adoption. Physical custody of the child should should not be returned to the parent legal guardian agency for the following reasons:
