If you are requesting a dismissal of a Summons for "Driving without Insurance or Driving without proof of Insurance" because you had a valid insurance policy for the vehicle on the date of the violation, you must submit an original letter from your insurance company or agent on their letterhead. No faxes, copies of the letter or an insurance policy will be accepted. The letter must include the following information: · · · · Policy number Description of motor vehicle covered under policy with VIN number. Effective dates of policy. A statement that the vehicle was insured on the specific date of violation.
