Instructions for Completing Court Order Restriction of Driving Privileges Tobacco Prevention Act - Minor

These instructions are being provided to help the court complete Form VS-200. The document must be completed in its entirety for the Driver Records Office to process it. This document should only be used to impose driving restrictions under Section 16-17-500 (E)-(4). The information requested should be typed or printed clearly. If the information requested is not provided, Driver Records will return the document to the court for completion. Section 1 ­ Judge's Name This section identifies the judge who is requesting DMV to restrict the minor's driving privileges. Section 2 ­ Minor's Information This section must be completed in its entirety. List the minor's name, address, city, state, zip code, driver's license or beginner's permit number, and date of birth. Also list the legal guardian's name, address, city, state and zip code. Section 3 ­ Restriction Information This section should be used to identify the restrictions imposed by the judge. The effective date and termination dates should only be indicated if the minor is the holder of a driver's license or beginner's permit. When the minor is not licensed, the issuance of a credential will be delayed if the minor makes an application before his or her 18th birthday.

Boxes 1 ­ 4 Driving Restrictions Use only if the minor is the holder of a beginner's permit or driver's license. In this area the court may mark one or all four of the driving restriction boxes. Box 4 If box 4 is selected, the court must provide under the area of Additional Instructions what the driving restrictions are. Box 5 This box should only be selected when the minor is not a licensed driver or the holder of a beginner's permit. The DMV will create a customer record for the court ordered restriction and will automatically impose the restriction if the applicant makes an application for a credential before his or her 18th birthday. Additional Instructions This area may be used to list any special restrictions the court imposed on the minor (box 4 should also be checked).

Section 4 ­ Court Information This section is used for the court signing off on the driving restrictions imposed. Provide the date, judge's signature, court name, and court address. When a court later determines that modifications are needed to the Form VS-200, the court is required to submit a court order signed by the same judge.
