ARTHUR G. CONNOLLY, IH, after tirst being duly sworn, on this 29*% day of
December, 2005, does depose and say:
1. I am an attorney with the law firm of Connoliy Bcve Lodge & Hutz LLP and
represent piaintiff in the above captioned action.
2. On November 29, 2005, I caused a copy of the Complaint, the Summons for
Defendant Alticor, Inc., and a letter addressed to Alticor, Inc., and containing the infomation
required by I0 Q; Q; § 3104, to be forwarded, via Registered Mail Return Receipt Requested,
to defendant Aiticor, Inc., pursuant to 10 Qel, WCW, § 3l04. The Registered Mail receipt for said
package is attached hereto as Exhibit "A".
3. On December 5, 2005, the package referenced in. paragraph 2, was received by
Aiticor, Inc. The contimiatiori/tracking form received from the United States Postal Service is
attached hereto as Exhibit "B”.
