The Debtor, student, supervising attorney, and faculty member, by executing this certificate below, respectfully represent as follows: 1) The Debtor (s) has been referred to the Bankruptcy Clinic at the Roger Williams University School of Law Metropolitan Center in Providence, Rhode Island (the "Program") and hereby consents to the assistance and special admission of , a third year law student enrolled in the Program in his/her/their case.
