CIVIL JURY ROSTER ATTACHMENT FOR CASES BEFORE THE HONORABLE MATTHEW J. PERRY, JR.
TO: ALL COUNSEL AND/OR PRO SE PARTIES ­ PLEASE TAKE NOTICE OF THE FOLLOWING: pJURY LIST: pJURY QUESTIONNAIRES:

Available 7 working days prior to jury selection day. You may obtain this list from the Clerk's Office ­ Jury Administrator. The court will submit written interrogatories to the jury venire for this term of court. Copies of the responses will be available beginning one week prior to jury selection day. They may be purchased at Franklin's Printing, 1114 Washington Street, Columbia, SC 29201 (phone: 803252-6310, fax 803-252-6242, or they will be available for review at the Jury Administrator's office in Columbia during the same period. THESE INTERROGATORIES WILL NOT BE REPEATED AT JURY SELECTION. Blank copies of the Questionnaires are available from the Jury Clerk and on our web site for you to use in preparation of your voir dire questions to avoid duplication. Pursuant to Local Rule 47.02, attorneys and parties must comply with the procedure for inspecting or purchasing Juror Questionnaires. You must make your request on a "Juror Questionnaire Request Form." You can obtain additional forms from the Jury Administrator at the Clerk's Office. After completion, a deputy clerk must approve it, and the Clerk will keep the original in the venire file and return the copy to you. You must present the approved form to the printer to purchase the questionnaires. The information contained in the questionnaires is to be used solely for evaluating potential jurors for jury service for your case and is not to be used or distributed for any other purpose. Any person wanting to obtain this information for any other purpose must petition the court for a hearing. If you have any questions about this procedure, please cal the , Jury Clerk at 765-5584.

pTRIAL BRIEFS:

Trial briefs are to be submitted (original and one copy) in all cases set for trial, five days prior to the opening of the session of court, in accordance with Local Rule 26.05. If your case has appeared on a prior trial roster and you have previously submitted a trial brief, you must resubmit your trial brief for this term of court.

pPROCEEDING BEFORE A UNITED STATES MAGISTRATE JUDGE: In accordance with the provisions of 28 USC § 636(c), you are notified that the United States Magistrate Judges of the District Court, in addition to their other duties, may, upon consent of all parties in a civil case and with the approval of the District Judge, conduct any and all proceedings, including a jury or non-jury trial, and order the entry of a final judgment. The completed consent form must be submitted to the Clerk's Office NO LATER THAN one week prior to jury selection day. pMEDIATION: Parties are hereby advised that if they are inclined to submit these matters to mediation, you must submit a copy of this court's Mediation Initiation Form. If mediation is to be attempted, parties should submit this form to the Clerk of Court (per the form's instructions) NO LATER THAN one week prior to jury selection day.
