! The Petitioner is under the age of sixteen (16) and: ! Has the consent of each parent capable of giving consent, OR ! Has the consent of his or her guardian, OR ! The Petitioner's parents are not living together and the Petitioner has the consent of his legal
custodian or the parent with whom he/she is living with. WHEREFORE, for the reasons set forth above, the Petitioner respectfully requests that the Court order the County Clerk and Recorder to issue a marriage license and marriage certificate form.
