I have been advised and understand that I am entitled bylaw to a hearing and assistance of
counsel before any unfavorable change may be made in my Conditions of Probation and
Supervised Release or my pqriod of supervision being extended. By ‘assistance of counsel', I
understand that 1 have the ri t to be represented at the hearing by counsel of my own choosing if
lam able to retain counsel. False understand that I have thc right to request the court to appoint
counsel to represent me at such a hearing at no cost to mysclfifl am not able to retain counsel of
my own choosing.

1. The defendant shall articiphte in sex oifcnder assessment and trcntment, including but not
limited to polygrapi plcthysmograph examinations recommended by the treatment
provider and/or dircptcd by the probation officer. The defendant shall contribute to the
cost as determined the U.S. Probation Ofiiccr.
2. The dcfcndant shall ot consume any alcoholic beverages.
3. The dci`cndant’s resi ence qnd employment shall be pre-approved bythe probation ofiiccr.
4, The defendant shall but associate with children who appear to be under the age of 18 nor
{`rcquent, volunteer, ior work at places whcrc children congregate (eg. playgrounds, parks.
malls, day-cure oentprs, or itchoois) unless approved by the probation ofiiccr. The
defendant shall havq no contact with victims.
5. The defendant shall ot possess, view, listen to, or go to locations whcre any form of
pornography, sexuany stimitlating material, or sexually oricntcd material is available. The
defendant shall not possess or use a device capable of creating pictures or video.
6. The detbndant shaI\‘not rent or use u post office box or storage facility.
7. The defendant shallisubmit. his or her person, and any property, house, residence, vehicle,
papers, computer, ther electronic communication or date storage devices or media, and
effects to search at ny time, with or without a warrant, by any law enforcement ofiicer or
probation oflicer w h reasonable suspicion concerning a violation of a condition of
probation, supcwis rclcasc, or unlawful conduct by tho person, and by any probation
oFEccr in thc lawiinl discharge ofthe ni‘Licer's supervision timctions.

which may include 30-dey inpatient treatment program followed by a 90 days in tx

10. The detiendant shall pay restitution to the vietim(s) identitied in the prescntenee report in
an amount totaling $7,382.00. Payments shall be submitted to the Clerk, U.S. District
Court. Should therelbc an unpaid balance when supervision commences, the defendant
shall pay the remainihg restitution at a minimum monthly rate of 10 percent ofthe
dct`cndant’s monthlylgross monthly income. Pursuant to IB U.S.C. §3664(k), the
defendant shall notily the Court and United States Attorney of any material change in
economic circums 'ces that might affect ability to pay. Restitution will he joint and
several with Alvinlllllabinson and Matthew Entzminger.
