Upon consideration of the Petition for Appointment of Guardian for the above Ward and hearing on ________________________ (date),

The Court finds:

That there is clear and convincing evidence that the Respondent is an incapacitated person and the Respondent's needs cannot be met by less restrictive means, including the use of appropriate and reasonably available technological assistance. 3. The Guardian does not have the authority to obtain hospital or institutional care and treatment for mental illness, developmental disability or alcoholism against the will of the Ward pursuant to §15-14-316(4), C.R.S. The powers and duties of the Guardian are unrestricted. The powers and duties of the Guardian are limited by the following restrictions:

The Guardian may not establish or move the Ward's custodial dwelling outside the State of Colorado without a Court order pursuant to §15-14-315(1)(b), C.R.S.

2. The Guardian shall notify the Court within 30 day if his/her home address, email address, or phone number changes. 3. The Guardian may manage the day-to-day finances for the support, care, education, health and welfare of the Ward. The Guardian is required to maintain supporting documentation for all receipts and all disbursements during the duration of this appointment. The Court further orders the following:
