Prescribed by Department of Local Government Finance STEP 1 - PETITIONER'S INSTRUCTIONS This application is to be filed with the County Assessor after representatives of both the Farm Service Agency (FSA) (formerly ASCS) [STEP 2] and the Natural Resources Conservation Service (NRCS) (formerly SCS) [STEP 3] have verified that a wetland area of two and five-tenths (2.5) or more contiguous acres exists on the parcel described below. A separate application must be filed for each parcel that contains a wetland.

STEP 2 - FARM SERVICE AGENCY INSTRUCTIONS This application is being submitted to the FSA office for assistance in verifying to the assessor that a U.S.D.A. designated wetland area containing two and five-tenths (2.5) or more of contiguous acres exists on the above described agricultural parcel. Contiguous means not touching another wetland area. Please log-in the application and complete the FSA farm and tract number areas, or insert N/A if the person is not considered a U.S.D.A. participant. Forward the application to the NRCS office for review.

STEP 3 - NATURAL RESOURCES CONSERVATION SERVICE INSTRUCTIONS The petitioner filing this application is asking the Natural Resources Conservation Service to verify that there is a U.S.D.A. certified wetland on this parcel. To correctly credit the wetland adjustment to the individual property description, the assessor must know the measurement of wetland acreage by individual soil map units and the type of wetland area. Types of wetland areas as defined for assessment purposes are: Type 43 indicates that the land area has been designated by the U.S.D.A. as farmed wetlands and contains at least two and five-tenths (2.5) or more acres of contiguous* land. indicates that the land area has been designated by the U.S.D.A. as wetlands and contains at least two and five-tenths (2.5) or more acres of contiguous* land. *Contiguous means not touching another wetland area. Type 73
