The purpose of this form is to identify all of the source specific operating agreement requirements for 326 IAC 2-9-13. Detailed instructions for this form are available online at www.in.gov/idem/permits/air/apps/instructions/oa13instructions.html. Rule specific definitions are listed in 326 IAC 2-9-13(a) for boiler, dryer, oven, process heater, space heater, and water heater. All information submitted to IDEM will be made available to the public unless it is submitted under a claim of confidentiality. Claims of confidentiality must be made at the time the information is submitted to IDEM, and must follow the requirements set out in 326 IAC 17.1-4-1. Failure to follow these requirements exactly will result in your information becoming a public record, available for any one to inspect and photocopy.

Part C: Compliance Option Part C identifies the option that source is choosing for compliance with the requirements of 326 IAC 2-9-13. Choose only ONE of the following options.

OPTION 1: I will comply with the requirements of 326 IAC 2-9-13 by carrying out the following set of requirements for any external combustion source, including any combination of boilers, space heaters, ovens, dryers, or water heaters.
Process Limitation: 326 IAC 2-9-13(b) (1) and (2)(A) · · Visible emissions from the source shall not exceed twenty percent (20%) opacity in twenty-four (24) consecutive readings in a six (6) minute period. The opacity shall be determined using 40 CFR 60, Appendix A, Method 9*. Limit fuel usage for every twelve (12) month period to less than the limits found in subsection (f), Table 1 for a single fuel or a combination of two (2) fuels.

Monitoring Requirements: 326 IAC 2-9-13(c) Sources must be able to demonstrate compliance no later than thirty (30) days after receipt of a written request by the department or U.S. EPA. No other demonstration of compliance shall be required. A source specific operating agreement is not required for these sources.

OPTION 2: I will comply with the requirements of 326 IAC 2-9-13 by carrying out the following set of requirements for any external combustion source, including any combination of boilers, space heaters, ovens, dryers, or water heaters.
