MISSING FILL AND/OR VAPOR CAP(S): The observed violation of 326 IAC 8-4-6 is missing fill and/or vapor cap(s) on the underground storage tank(s). In the future, please ensure that all fill and/or vapor caps are present, latched, and form a snug fit over the gasoline fill and vapor return tubes. LOOSE OR DAMAGED FILL AND/OR VAPOR CAP(S): The observed violation of 326 IAC 8-4-6 is loose or damaged fill and/or vapor cap(s) on the underground storage tank(s). In the future, please ensure that all caps are latched and form a snug fit over the gasoline fill and vapor return tubes. MISSING OR DAMAGED GASKET(S): The observed violation is 326 IAC 8-4-6 is missing or damaged gasket(s) on the fill and/or vapor cap(s). In the future, please ensure that the gaskets on all caps are in good working condition so that the caps can form a snug fit over the gasoline fill and vapor return tubes. MISSING PRESSURE VACUUM (P/V) VALVE(S): The observed violation of 326 IAC 8-4-6 is missing pressure vacuum valve(s). In the future, please ensure that pressure vacuum valves are present and in good working condition so that gasoline vapors are not release to the atmosphere. FAILURE TO INSTALL STAGE I: The observed violation of 326 IAC 8-4-6 is the failure to install Stage I Vapor Recovery.
