WELL PERMIT INFORMATION, PERMITS APPROVED PURSUANT TO SECTIONS 37-90-107 AND 37-90-111(5), C.R.S. (DENVER BASIN AQUIFER WELLS LOCATED IN DESIGNATED GROUND WATER BASINS) -RESIDENTIAL USE WELL PERMIT ­ ISSUED PURSUANT TO A REPLACEMENT PLANPLEASE NOTE CAREFULLY THE CONDITIONS OF APPROVAL ON THE ATTACHED COPY OF YOUR PERMIT. THE CONDITIONS MUST BE COMPLIED WITH IN ORDER FOR THE PERMIT TO BE VALID. THE WELL CONSTRUCTION WORK MUST BE COMPLETED PRIOR TO THE EXPIRATION DATE OF THE PERMIT. THE PERMIT EXPIRATION DATE IS ONE YEAR FROM THE DATE ISSUED. Evidence of well construction must be submitted to this office in the form of a well construction report from the respective contractors showing that construction was completed prior to the expiration date of the well permit. The well permit number is located in the upper right corner and the expiration date is located in the lower right corner. At the discretion of the Commission, the expiration date may be extended once for up to one year if good cause is shown. If an extension of time is necessary to construct the well, you must file a written request with this office prior to the expiration date. The request must be received with a $60 filing fee and must state why the well has not been completed, must include an estimate of time required to complete the well, and must specify the length of extension you desire, not to exceed one year. A NOTICE OF COMMENCEMENT OF BENEFICIAL USE (FORM NO. GWS-19) MUST BE FILED WITH THE DIVISION OF WATER RESOURCES BY THE WELL OWNER WITHIN 30-DAYS AFTER FIRST COMMENCEMENT OF USE. Please retain the enclosed copy of this form and file it with this office when the well is
first put to use.

A TOTALIZING FLOW METER SHALL BE INSTALLED ON THE WELL AND MAINTAINED IN GOOD WORKING ORDER SO AS TO PROVIDE A CONTINUOUS AND ACCURATE RECORD OF WITHDRAWALS. THE WELL MUST BE CONTRUCTED AND THE PUMP INSTALLED BY CONTRACTORS WITH CURRENT LICENSE(S) ISSUED BY THE STATE OF COLORADO. The well construction report must be submitted to the
office of the State Engineer within sixty (60) days of completion of the work or within seven days after the expiration date of the permit, whichever is earlier. The pump installation report must be submitted within sixty days of completion of the pump installation work. Your contractor must provide you with a copy of the work reports they have filed with the State Engineer. The Well Construction and Test Report, Form No. GWS-31, and the Pump Installation and Test Report, Form No. GWS-32, are available from the Division of Water Resources Offices and the web site, www.water.state.co.us . (Note: Wells in some areas may be constructed by the well owner. Contact this office for more information.) The original permit is on file in the Denver Office Records Section. You have been provided with at least two copies of the well permit. One copy is for your records. The second is for the pump installation contractor. Permit copies have also been provided to the well construction contractor indicated in the application. If you did not indicate a well construction contractor on the application, four copies of the permit are enclosed. You may make additional copies of your permit copy for a well construction contractor if different from the one indicated in your application. Additional permit copies may be obtained from the Records Section, (303) 866-3447, for a fee of $3.00. Any change of mailing address or ownership must be reported to the State Engineer by the new owner on a Change in Ownership/Address form, GWS-11.
