INSTRUCTIONS FOR COMPLETING AN ORDER OF PATERNITY
COMPLETE THIS FORM TO REQUEST AN ORDER OF PATERNITY IF: · You completed a Request for Order of Paternity.

THE FORM SHOULD BE TYPED OR PRINTED NEATLY USING BLACK INK. TO COMPLETE THIS FORM YOU WILL NEED: · A copy of your Request for Order of Paternity. NOTE:

SOCIAL SECURITY NUMBER MUST BE PROVIDED. IT CAN BE OMITTED FROM THIS DOCUMENT AS LONG AS YOU INCLUDE IT ON THE CONFIDENTIAL SENSITIVE DATA FORM (ATTACHED).

The complete name, address and phone number of the person filing the form. (section "a") The name of each of the children for whom you wish to have an order of paternity entered. (section "b") Enter the name and social security number of the natural mother of the child(ren) as listed in the Request for Order of Paternity. (section "c") (*OMIT SOCIAL SECURITY NUMBER IF USING CONFIDENTIAL SENSITIVE DATA FORM) Enter the name and social security number, date of birth and place of birth of the natural father of the child(ren). (section "d") (*OMIT SOCIAL SECURITY NUMBER IF USING CONFIDENTIAL SENSITIVE DATA FORM)

WHEN YOU HAVE COMPLETED THIS FORM: · The Clerk will sign and date the Order. You may obtain a certified copy of the Order for a fee.

COPY OF NEW ARIZONA BIRTH CERTIFICATE: Arizona birth certificates are maintained by the Office of Vital Records, Department of Health Services, 2727 W. Glendale Ave., Phoenix AZ 85051. If you want a copy of the NEW Arizona birth certificate, allow two weeks for the change to be processed. Then, contact the Office of Vital Records. They will prepare the new certificate upon: · · · Request; Payment of a fee; and, Presentation of personal information. (Be sure to have a copy of the Order of Paternity with you)
