*If the Guardian was also appointed Guardian of the Estate of the Disabled Person, a separate Current Report regarding the Estate must be filed setting forth all receipts and expenditures since the Guardian was appointed as well as a statement of all assets belonging to the Disabled Person. This form must be submitted with $25.00 filing fee to the Clerk of the Court.
