I, Donald J. Amaral, declare as follows:
1. I submit this Supplemental Declaration in support of Defendants’
Motion to Transfer Venue to the District of Colorado. The following facts are
within my personal knowledge and, if called and sworn as a witness, I could and
would competently testify thereto.
2. I am a former member of the Board of Directors (the "Board") of
Coram Healthcare Corporation and its wholly owned subsidiary Coram, Inc.
(collectively, “Coram"). I am not currently a director of the reorganized Coram,
Inc.
3. I recall that the Board held approximately fifty percent (50%) of its
meetings in Colorado while I was a member.
4. The meeting at which the Board decided to hire Daniel D. Crowley
as Coram’s Chief Executive Officer was held in Colorado.
I certify under penalty of perjury under the laws of the United States that
the foregoing is true and correct.
