Plaintiff, United States of America, makes application in accordance with 28 U.S.C. § 3205(b)(1) to the Clerk of the United States District Court to issue a Writ of Garnishment upon the judgment entered against Defendant, Anne Elizabeth Huennekens, SSN XXX-XX-5489, whose last known address is: 4681 S. Decatur Street, #221, Englewood, CO 80110, in the above cited action in the amount of $2,740.25 (Special Assessment - $100.00; Restitution - $2,640.25), from May 15, 2001, until paid. A balance of $2,117.25 remains outstanding.

Demand for payment of the above-stated debt was made upon the debtor not less than 30 days from the date of this application, and debtor has failed to satisfy the debt.
