Gallegos, SSN XXX-XX-4377, whose last known address is: 5155 Tejon Street, Denver, CO 80211, in the above cited action in the amount of $3,100.00 (Special Assessment - $100.00; Fine $3,000.00) until paid. A balance of $1,300.00 remains outstanding. Demand for payment of the above-stated debt was made upon the debtor not less than 30 days from the date of this application, and debtor has failed to satisfy the debt. The Garnishee is believed to owe or will owe money or property to the judgment debtor, or is in possession of property of the debtor, and said property is a nonexempt
