Petition on Probation COMES NOW, Emily Valentin, probation officer of the court, presenting an official report upon the conduct and attitude of Tamara Basanilovna Borukhov who was placed on supervision by the Honorable Phillip S. Figa sitting in the court at Denver, Colorado, on the 8th day of April, 2005, who fixed the period of supervision at three (3) years, and imposed the general terms and conditions theretofore adopted by the court and also imposed special conditions and terms as follows: 1. The defendant shall not incur new credit charges or open additional lines of credit without approval of the probation officer, unless the defendant is in compliance with the installment payment schedule. The defendant shall provide her probation officer with access to any requested financial information, to include authorizing the probation officer to conduct regular credit checks and obtain copies of her federal income tax returns. The defendant shall not be self-employed, nor shall she be employed by friend, relatives, associates or persons previously known to her, unless approved by the U.S. Probation Office. The defendant shall not use, for any purpose or in any manner, nay name other than her true, legal name of Tamara Basanilovna Borukhov. The defendant shall not apply for renewal or replacement of her driver' license, social security card, s passport, or other form of identification without prior approval of her probation officer. The defendant shall pay a $2,500.00 fine.

ATTACHMENT Attached hereto as Exhibit A and incorporated by reference is a true copy of the Conditions of Probation signed by the defendant on April 8, 2005. Her signature on that occasion acknowledged that the conditions had been read and explained to her, that she fully understood said conditions, and that she was provided a copy of them. The term of probation commenced April 8, 2005. The petition is based on the following facts: The Court ordered that the Probation Department schedule a compliance review hearing.
